Once you are finished this form will be emailed to our reception and your membership will be reviewed. On arriving at the centre you should follow any COVID safety restrIctions in place, use the anti-bacterial gel in main reception entrance and queue. Please ensure you follow Government social distancing guidelines and help us keep Creation open, and safe.

At reception inform the receptionist you have completed an online registration form and they will locate your membership request.

Participation Statement: I fully understand and acknowledge that there are risks and dangers associated with participation in skateboarding, BMX, Scootering and Inline skating which could result in injury; partial and/or total disability, paralysis or death. As a result of this knowledge, I will not hold the management, directors or staff of Creation Skatepark responsible for any accident, injury or loss/damage whilst on these premises. By signing this membership form I confirm that I have read and understood the conditions of use and acknowledge that Creation Skatepark recommends the use of full protective equipment, and that helmets are compulsory at all times whilst riding. I agree to accept full responsibility for my child during their participation at Creation Skatepark.
